Position Summary:

The Licensed Practical Nurse (LPN) provides essential nursing care within a Residential Crisis Stabilization Unit, delivering 24/7 support to individuals experiencing acute behavioral health crises. Under the supervision of a Registered Nurse (RN), Nurse Practitioner, or Physician, the LPN ensures compliance with DBHDS regulations, focusing on medication management, clinical documentation, vital signs monitoring, coordination of care, and nursing assessments.

Key Responsibilities:

· Medication Management

* Administer medications as prescribed, adhering to established protocols and document in EMR.

* Monitor and document medication effects, side effects, and adverse reactions in EMR.

* Maintain accurate medication records, ensuring compliance with regulatory standards.

* Clinical Documentation

* Record all nursing interventions, assessments, and observations in the individual's electronic medical record.

* Document vital signs, medication administration, and any changes in the individual's condition.

* Ensure documentation meets DBHDS requirements for accuracy and timeliness.

· Vital Signs Monitoring

* Obtain and record vital signs upon admission, during service provision as per medical orders, and at discharge.

* Document any refusals and follow-up actions taken, in accordance with facility procedures.

* Perform routine physical examinations (e.g., skin checks, respiratory status, heart and lung auscultation).

· Coordination of Care and Follow-Up

* Collaborate with interdisciplinary team members to develop and implement individualized service plans.

* Assist in coordinating referrals and follow-up care, ensuring continuity of services post-discharge.

* Participate in discharge planning, providing necessary information and education to individuals and their support systems.

· Nursing Assessments

* Conduct nursing assessments on the day of admission, evaluating non-psychiatric medical conditions that may impact the individual's psychological state or ability to participate in services.

* Screen for communicable diseases, including tuberculosis, as required.

* Monitor and record pain levels, reporting significant findings and changes to the NP and Clinical Director.

* Observing and recording any changes in the patient’s condition or health status.

* Assessing mental and emotional status, ensuring patient safety and comfort.

* Document assessment findings, follow-up actions, and any necessary referrals.

· Compliance and Safety

* Adhere to all DBHDS regulations and facility policies regarding crisis services.

* Implement standard precautions and infection control measures to prevent the spread of communicable diseases.

* Maintain a safe and therapeutic environment for individuals receiving services.

* Provide patient education on general health topics, medications, and home care instructions when necessary.

* Maintain patient confidentiality and comply with all healthcare regulations (HIPPS, PHI, etc.).

Qualifications:

* Licensure: Current and valid Licensed Practical Nurse (LPN) license issued by the Virginia Board of Nursing.

* Experience: Prior experience in behavioral health, crisis stabilization, or related settings preferred.

· Skills:

* Strong clinical assessment and critical thinking abilities.

* Proficient in documentation and electronic medical record systems.

* Effective communication and interpersonal skills.

* Ability to work collaboratively within a multidisciplinary team and independently as required.

Work Environment:

The LPN will work in a residential facility providing crisis stabilization services, requiring flexibility to work various shifts, including nights, weekends, and holidays, to ensure 24/7 coverage. The role involves direct interaction with individuals experiencing acute behavioral health crises, necessitating a calm and supportive approach.
